Withdrawal Form For Camp

Any and all cancellations will require a minimum of two weeks notice in order to receive a full refund minus a $50 administrative fee. Any cancellations within two weeks of the enrolled program will not be eligible for any refunds or credits unless illness or injury with a doctor’s note. The doctor’s note must be given to HOH staff within two weeks of the program cancellation in order to receive the full refund minus the $50 administration fee.

Please include each camp date you wish to withdraw your child from.

By submitting this form, you are requesting that Head Over Heels drop your child(ren) from the camp dates listed. You understand that your child’s spot will no longer be reserved. You agree to pay any and all fees that may have accrued during your time with Head Over Heels including unpaid tuition, late fees, administrative fees, etc.
